Our Residential and Periodic services include those through MR/MI, CAP-MR/DD, and Medicaid.

 

MR/MI Services and Definitions

 

Personal Assistance

This is a support service which involves assistance to the individual so he or she can function in more normal environments. Areas for assistance include dressing, eating, taking medications, and personal hygiene.

 

Social Inclusion

This service provides strategies and activities that support personal interaction and enhanced social roles for community membership.

 

Supervised Living

This service provides residential placement in an unlicensed facility and provides up to 24 hours per day of direct supervision. Assistance and training for daily living skills are tailored to meet the needs of the individual.

CAP-MR/DD Services and Definitions

 

Crisis Stabilization

This is a more intensive level of intervention, providing close one-on-one supervision, involving interventions developed and approved by a mental health professional. This service is designed to reduce the need of inpatient hospitalization, while ensuring for the individual's safety and well-being.

 

Day Habilitation

This service provides training in the acquisition, retention, and improvement in self-help, socialization, and adaptive skills, occurring on a regular basis, in a non-residential setting.  

 

Respite

This service provides the individual's primary caregiver periodic relief on an interim basis, and is provided in the home and community.

 

Supported Employment

This service provides a supervised setting for the individual to work in the community, as well as providing training specific to work related skills.

 

Residential Supports

This consists of supports that promote the acquisition, retention, or improvement in skills for greater independent living. Training includes area such as self-care, communication and skills participating in the community.

 

Home and Community Supports

This consists of periodic supports that promote the acquisition, retention, or improvement in skills for greater independent living. Training includes area such as self-care, communication and skills participating in the community.

 

Specialized Consultative Services

This provides services in a specialty area, such as psychology, speech therapy, therapeutic recreation, or nutrition. This service is designed for individuals needing intensive consultation because of extreme or complex medical, psychological, or psychiatric needs.

 

Personal Care

This service provides assistance to the individual for activities of daily living, such as personal hygiene, eating, dressing, and medication administration.

Medicaid Services and Definitions

 

Community Supports - MH/SA Adult

Provided by qualified professionals and carried out in the home and community, this service focuses on supports for the rehabilitation skills and abilities affected by mental health issues. Designed to prevent any further deterioration of skills and functional abilities, CBS provides assistance and training so the individual may continue to live in the community.

Referral Process

 

The referral process involves collaboration with the Local Management Entity (LME) and contracted Case Management agencies. A representative from these organizations consults with CLC's Consumer Services Director to determine the individuals needs. Arrangements are then made to meet with the individual to discuss these needs further and to determine the types of needed services. Should the individual choose services with CLC, the individual and his or her treatment team determine the course of service delivery.
